In reviewing the definition of 21299 it is a surgical code that may not adequately be the correct code for a TMD appliance. Code D7880 may more accurately describe the procedure.

Great question. If the oral appliance is a removable intra-oral appliance being used to treatment temporomandibular joint disorders (TMJD), ever since the S8262 code discontinuation a few years ago, what we have found is that the two codes that are most commonly accepted by medical insurers for TMJD appliances are:
21299 - Unlisted craniofacial and maxillofacial procedure
D7880 - occlusal orthotic device, by report

A narrative report explaining the treatment accompanying the claim is recommended since they are "by report" and “unlisted” codes

What medical code is Insurances using being use or accepting for Occlusal guard? I have heard of 21299 or 20999 and HCPCS Code D7880.
