In reviewing the definition of 21299 it is a surgical code that may not adequately be the correct code for a TMD appliance. Code D7880 may more accurately describe the procedure.

If the oral appliance is a removable intra-oral appliance being used to treatment temporomandibular joint disorders (TMJD), ever since the S8262 code discontinuation a few years ago, what we have found is that the two codes that are most commonly accepted by medical insurers for TMJD appliances are: 21299 - Unlisted craniofacial and maxillofacial procedure D7880 - occlusal orthotic device, by report
A narrative report explaining the treatment accompanying the claim is recommended since they are "by report" and “unlisted” codes
