Great question. Typically medical insurance does bundle those into the codes you mentioned - 21248 & 21249.

Good morning! I have a patient that had an implant placed using code D6115 but we are billing to Medicare and need to change it to a medical code. I saw on a couple of other posts the use of 21248 and 21249 but those are reconstructions for D6010. What should be used for the actual implant?
