Patient fainted and fell forward hitting upper teeth on a hardwood floor. Impact resulted in one front tooth avulsing up into the maxilla. Initial consult with general dentist was delayed due to Covid quarantine period.
Initial consult with general dentist resulted in referral for orthodontics to pull tooth back into position. Some movement occurred before root became ankylosed.
Patient referred to periodontist and prosthodontist.
Periodontist services were performed as follows. Medical insurance requires CPT code or codes for dental procedures due to an accidental injury in order to file a medical claim. D4266 - CPT code? D7210 - CPT code? D7953 - CPT code?

Patient asked each dental provider to file medical claim but was told that they only file dental claims and did not have CPT codes to file medical claims. Medical insurance requires a CPT code for "accidental dental" but will not provide this code to the patient. There may be only one CPT code needed for "accidental dental" that can be used for all dental procedures. The patient needs assistance in locating the CPT code for accidental dental or accidental dental injury.
