CPT has added botox codes for the facial area:

64612 CHEMODENERVATION OF MUSCLE(S); MUSCLE(S) INNERVATED BY FACIAL NERVE, UNILATERAL (EG, FOR BLEPHAROSPASM, HEMIFACIAL SPASM)

64612 would be used for jaw dyskenesia (dystonia)



64615 CHEMODENERVATION OF MUSCLE(S); MUSCLE(S) INNERVATED BY FACIAL, TRIGEMINAL, CERVICAL SPINAL AND ACCESSORY NERVES, BILATERAL (EG, FOR CHRONIC MIGRAINE)

There is no guarantee that the insurance will reimburse for botox. These treatments are starting to be more common. Botox will not be reimbursed for cosmetic purposes.
