Hi rwcottrell!

The diagnosis code you will want to provide to the insurer will be whatever condition the scan are being taken for - i.e. if you are fabricating an oral appliance for obstructive sleep apnea, the diagnosis code you would provide is 327.23.

As for your pre-auth for 76376, Tracy will contact you to get that clarified!

I am tring to get phone auth for ct scan codes 70486 and 76376, insurance requires 76376 to be preauthorized, on the preauth from given from Tracy she doesn't have diagnosis codes on them however the insurance company says they have to have to give the auth, how can i get anthem to approve the ct scans
