The cost of oral appliances therapy to the patient will depend on what services you provide as part of treatment, and the oral appliance used may effect the cost of treatment, depending on what the lab fee is for the appliance you choose.

Having said that, approx $2500 is the average fee for the consult, impressions & bite, appliance itself, and the delivery & adjustment (approx 90 days follow up care). Fee range widely from about $1500-$6000 for the therapy depending on the provider (as well as the allowed amounts for the services set by the insurers). Medicare DME jurisdiction D's allowed amount for E0486 is set between about $1300-$1500

If you are separating the cost of the appliance itself to the patient from the rest of the treatment, the fee for the appliance (code E0486) will generally range from approx. $1000-$2000.
