Hi Jane! Great question.

If you are submitting an electronic claim there will generally be a check box for you to select within the clearinghouse's website that indicates the claim is secondary. indicate the total amount paid from the EOB from the primary insurer in the "amount paid" box in the CMS1500 (box 29)

If you are submitting a paper claim, indicate the total amount paid from the EOB from the primary insurer in box 29, and send a copy of the primary payor EOB in with the claim.

I have a claim to UHC,sec. medical insurance, which has already been paid by primary dental policy. How do I indicate this on the claim form? Thanks, Jane Ellen Teller
