There are multiple places to store values for the CMS1500 Medical Claim form.

First, there is a section under the TOOLS / Options / Claim Forms where you can enter the Tax ID, Doctor's signature, the Facility Address and associated NPI number, the Billing Facility address and associated NPI number and whether you accept assignment of not.


Secondly, you can enter the patients insurance company name and insured information by selecting the patient, the selecting the "Insurance Tab" and completing the form shown. This information will be used later when creating the claim form.


Lastly, you can create Claim Form Templates that will store the Diagnosis and Procedure codes. The templates are created from the actual Claim Form by selecting the "Bulleye" icon on the top bar of the claim form and giving it a unique name for the claim template.
