Here are some excision of lesion CPT coding options with their descriptions:

11440 excision benign lesion 0.0 to 0.5 cm
11441 excision benign lesion 0.6 to 1.0 cm
21030 removal of facial bone lesion benign
21040 removal of lesion mandible benign simple
40810 excision of lesion, vestibule
40812 excise/repair mouth lesion
40814 excise/repair vestibule lesion
40816 excision of mouth lesion complex w/muscle
41116 excision of lesion, floor of mouth

Whichever one most closely describes the procedure you performed is the best one to go with!

As for bone grafting:

21210 - Graft, bone; nasal, maxillary or malar areas (includes obtaining graft)
21215 - Graft, bone; mandible (includes obtaining graft)
**use modifier -52 when bone not obtained from patient

What CPT code(s) would I use for D7411 - excision of benign lesion between #8-#9?

I am trying to decide between 40814 or 21030.

Also, what is the CPT code for debridement and bone grafting of periodontal defect?
