Dr. Sterling,

Most companies now request a letter for the preauthorization requesting the treatment. Also, check the insurance companies website for a predetermination form for that company.

Insurers have said they would like to have the dates of service blank, however they seem to get confused and deny
the claim due to no date of service.

It’s best to call or visit their website to find out how to submit a predetermination for that particular insurer.

How do I create a predetermination/claim without a date of service? Many thanks!
