Can anyone please help with the CPT code(s) for wisdom teeth extractions (D7220, D7230, D7240, D7241, D7250)

As for the CPT code for extractions, there is actually not a direct crosscode, so you can either bill the "D" code on the medical claim (many insurers these days will process "D" codes when they are medically necessary services), or you can use the CPT code below and include a narrative report describing the procedure: 41899 - Unlisted procedure, dentoalveolar structures.

Are there diagnostic codes that would have to be added (for these extraction codes) if we are just submitting the "D" codes or no, since they are dental codes?

Yes you will definitely want at least 1 diagnosis code for any medical claim you send, even when using the "D" codes :)
Now, the diagnosis code(s) to use will depend on the patient's condition, or *why* the teeth are being extracted. For example, you may use one of the following below if they apply:
K01.1 - Impacted teeth K01.0 - Embedded teeth M26.31 - Crowding of fully erupted teeth
If the teeth are being extracted for a different reason, let me know and I am happy to offer you some coding options.
