D9940 D9940 has been deleted from the coding system, replaced with the following 3 codes: D9944 occlusal guard – hard appliance, full arch D9945 occlusal guard – soft appliance, full arch D9946 occlusal guard – hard appliance, partial arch
If this code is being use to bill medical insurance for appliances being used to treat temporomandibular joint disorders, most practices we work with will code that as D7880, which stands for: occlusal orthotic device, by report Since this code has "by report" in the description, they can explain in the narrative report what the appliance is/does. Now, you will every so often run into a medical insurer that will not process any code starting with a "D" - most do these days, but not all. In those cases, you end up using the closed CPT code that is "by report" or "miscellaneous". The top options seem to be:
21299 - Unlisted craniofacial and maxillofacial procedure E1399 - Durable medical equipment, miscellaneous
D2740 - crown - porcelain/ceramic substrate D2750 - crown - porcelain fused to high noble metal
There are not a direct crosscodes we are aware of for these procedures, so you can either use the "D" code on the pre-authorization request/medical claim (many insurers these days will process "D" codes when they are medically necessary services), or you can use the CPT code below and include a narrative report describing the procedure: 41899 - Unlisted procedure, dentoalveolar structures

D9944 - occlusal guard – hard appliance, full arch
The diagnosis code(s) used will depend on the condition/symptoms, or in other words, why is the patient receiving the services?
For example, if the patient is receiving an occlusal guard due to bruxism, you may consider: G47.63 - Sleep related bruxism F45.8 - Other somatoform disorders
If you'd like to provide some information about the patient's condition/symptoms, I'm happy to offer you some coding options.

No problem. Just a heads up, most medical insurers do specify that they will cover accidental injury to "sound, natural teeth", so you may run into issues with coverage of the broken bridge - but the knocked out teeth may certainly be covered.
D2740 - crown - porcelain/ceramic D2950 - core buildup, including any pins when required D6740 - retainer crown - porcelain/ceramic D6245 - pontic - porcelain/ceramic D2954 - prefabricated post and core in addition to crown D2331 - resin-based composite - two surfaces, anterior
The codes listed above do not have direct crosscodes we are aware of, so you can either bill the "D" code on the medical claim (many insurers these days will process "D" codes when they are medically necessary services), or you can use the CPT code below and include a narrative report describing the procedure: 41899 - Unlisted procedure, dentoalveolar structures
You'll also need ICD-10 (diagnosis) codes to describe the injury & accident. Here are some common ones for motorcycle accidents:
S02.5XXA - Fracture of tooth (traumatic), initial encounter for closed fracture S02.5XXB - Fracture of tooth (traumatic), initial encounter for open fracture
K08.411 - Partial loss of teeth due to trauma, class I K08.412 - Partial loss of teeth due to trauma, class II K08.413 - Partial loss of teeth due to trauma, class III K08.414 - Partial loss of teeth due to trauma, class IV
G89.11 - Acute pain due to trauma
As for the diagnosis code to describe the accident, if you would like to reply with a few more details about the accident I am happy to offer you some coding options. I'll need the following details: - was your husband the driver or a passenger? - was there is a collision with another vehicle? If so, what kind? (car, truck, SUV, etc) - or, was there a collision with something else? (i.e. stationary object, animal, etc)

One of our patients in the dental office, has had a considerable amount of treatment due to cancer treatment. His dental policy has maxed and now we want to try to bill his medical insurance. Can the dental codes listed be converted to medical codes?
