D6010- surgical placement of implant body: endosteal implant
21248-implant endosteal, partial reconstruction 21249-implant endosteal, complete reconstruction
D4267- guided tissue regeneration non-resorbable barrier, per site
41899- dentoalveolar procedure unlisted ** include a narrative report describing the procedure
D7953 bone replacement graft for ridge preservation- per site
21215 graft, mandibular ** use modifier -52 for reduced services when bone is not obtained from patient

The diagnosis codes are going to vary depending on what the patient presented with.
Why does the Dr. feel the treatment is necessary? what did the pt. present with? (i.e. atrophy, fractured tooth, loss of tooth)

Thank you again for your respond. The pt. lost the tooth due to periodontal disease. I checked I could possibly use code what I list below to claim. Could I ask a silly question? What the class I, II, III, IV mean? So I could know which code is more accurate.
K08.12 Complete loss of teeth due to periodontal diseases K08.121 is a specific ICD-10-CM diagnosis code K08.121 …… class I K08.122 is a specific ICD-10-CM diagnosis code K08.122 …… class II K08.123 is a specific ICD-10-CM diagnosis code K08.123 …… class III K08.124 is a specific ICD-10-CM diagnosis code K08.124 …… class IV K08.129 is a specific ICD-10-CM diagnosis code K08.129 …… unspecified class

Classification System for the Completely Edentulous Patient Class I - Ideal or minimally compromised Class II - Moderately compromised Class III - Substantially compromised Class IV - Severely compromised
Diagnostic Criteria 1. Bone height - mandibular 2. Maxillomandibular relationship 3. Residual ridge morphology-maxilla 4. muscle attachments

D6104 - bone graft at time of implant placement can be cross coded to: 21210 - Graft, bone; nasal, maxillary or malar areas (includes obtaining graft) 21215 - Graft, bone; mandible (includes obtaining graft) **use modifier -52 when bone not obtained from patient
D4266 - guided tissue regeneration - resorbable barrier, per site
The code listed above have a direct crosscode we are aware of, so you can either bill the "D" code on the medical claim (many insurers these days will process "D" codes when they are medically necessary services), or you can try the CPT code below and include a narrative report describing the procedure: 41899 - Unlisted procedure, dentoalveolar structures
