I work for a dental office and we don't do many medical claims. I'm sending a medical claim for the implant abutment and crown but not sure which diagnosis code to use. I've found K08.4 and saw that it should be used as K08.411,412,413,414. I'm confused though because those range from Class I-IV but how do you know what the classes mean? I also saw K08.419 which is for an unspecified class, I'm thinking this is the one to use but not sure.
